Case management is the structured and coordinated design of support and counseling processes for people who are affected by problems in several areas of life and are dependent on the support or services of various specialized agencies. Case managers continuously support people in their long-term health, social and professional integration. They initiate and organize cross-case support networks in the health and social sectors, which are oriented towards the individual living environments, needs and requirements of their clientele. In this practice-oriented course, you will expand your profile and acquire case- and system-related action competencies in order to be able to work as a case manager in various fields of work in the health and social sectors. The course demonstrates new methods of professional action with complex cases and provides in-depth knowledge of the action concept of case management as well as basic knowledge in the field of network-oriented system management. As a participant, you will gain confidence in case management and the ability to work in a resource- and network-oriented manner. You will be able to support the necessary structural development and implementation of case management and apply what you have learned in your practical work in various settings.
The modular course (NDK) is aimed at qualified healthcare professionals and managers from acute hospitals, long-term institutions, Spitex organizations, rehabilitation clinics, insurance specialists and other institutions in the healthcare and social sectors.
